Today I would like to discuss a new client of mine Mr. Guy Generic who is 48 years of age. Mr. Generic (Guy) lost his job three months ago. Guy has a previous diagnosis of Bipolar Disorder for which he was taking a prescribed medication. Mr. Generic has told me that a few years ago he regularly attended group therapy of some sort, which coupled with his prescription medication seemed to keep him on an even keel. Due to his recent lack of finances and health insurance Guy has not been able to obtain his medication. Guy says that lately he has only been able to sleep two-three hours each night and has begun to experience auditory hallucinations along with some psychosis, as is symptomatic of Bipolar Disorder. Finding himself in these circumstances Guy has reverted to his old drug habit to alleviate the symptoms of his Bipolar Disorder, and to further complicate his problems Guy admits to engaging in unprotected and otherwise risky sexual acts and does not seem to care about his own hygienic and self-care needs. My client is in a real crisis situation so I have come to the team today with an initial plan of care for Mr. Generic, he is decompensating quickly and any constructive input you may have to assist me in helping him would be greatly appreciated.

So far I have referred Mr. Generic for a psychiatric evaluation as well as a medical health workup and am awaiting the results from those doctors. My initial or most immediate concerns for Guy are that he has not been treated for his Bipolar Disorder for three months which is wreaking havoc on every area of his life biochemically. The next thing which most concerns me for this client is that without a job he will soon become homeless as well, after three months without an income I wonder if he even has utilities. This situation is going from bad to worse, so I need you all to put on your thinking caps and pull out your super powers.

After much deliberation; with myself and then with Guy, we believe that the safest and most efficacious next step is to make certain that he is safe from himself as well as others. Mr. Generic has agreed to go inpatient for a temporary period, at least until he is medically stabilized again. During which time he will participate in a duel-diagnosis therapy group preferably one focused on persons diagnosed with Bipolar Disorder at some level. I chose the duel-diagnosis group over the others because for one thing Guy has been dealing with this his whole life and is quite aware of his condition and the symptoms of untreated Bipolar Disorder. For another thing I think him engaging in a group with others with the same diagnosis is most beneficial for Guy and the others in the group. (Croft, 2015) Quite frankly, I cannot see a downside or drawback to this choice of group therapy. While Guy is doing his part, I will get him signed up for County Health Care which will pay for his prescriptions and other health care needs. I will help him get signed up for unemployment which he has not done because he has not been able to make rational choices for himself during this crisis.

My back up plan for this initial stage is to contact our partners in community services and see what we can do about helping him to salvage his home and keep his utilities on at least until Guy is back to work. If it is necessary I will also see if he would benefit from a training program to assist him in sustaining himself.

Right this minute Guy needs to be safe because having been off of his psychotropic for so long has left him vulnerable even to himself, as he is incapable of making consistently rational choices at this time, which was made evident at the beginning of the meeting. The back-up plan then is to keep his home and utilities going for his return from his short in-patient stay here in County, also just in case a bed does not open up for him. My client Mr. Generic claims that he has no family to rely on accept an estranged sister who has not wanted to have anything to do with him since their parents died ten years ago, he never married or had any children that he is aware of, nor does he have any close friends to call upon for such things. I figure that once these initial objectives are met we can move on to the next stage.
